Resend PIX key OTP

Sends the OTP again for a pending email/phone key challenge. Rate-limited: 60 s cooldown between sends and at most 3 sends per challenge (resend_too_soon). If the provider fails to deliver the response is 503 otp_send_failed and the challenge is no longer usable — start over with POST .../pix/keys.
